El año pasado 260 menores de 14 años tuvieron bebé.
"Para algunos esto es un juego y dicen que no tienen porque hacer caso a lo que les digan sobre su sexualidad. Otros entendemos que debemos considerarlo porque así evitamos enfermedades y comprendemos que el amor no es solo sexo".
Así Carolina Valencia, estudiante del colegio República de Israel, asegura que la campaña 'Hablemos de sexualidad con enfoque en amor' de la Secretaría de Salud de Cali, es una oportunidad para que los jóvenes reciban información y sean escuchadas sus inquietudes sobre estos temas.
Y es que con estos conversatorios, las autoridades de salud buscan acercarse a los adolescentes para reducir los embarazos y trabajar planes de prevención de enfermedades de transmisión sexual.
Sonia Quebradas, directora de la Línea de Salud Sexual y Reproductiva, dice que este empeño implica generar espacios para ellos. En la última década se ha registrado un incremento de embarazos en menores de 18 años. Una de cada cinco embarazadas en Colombia es adolescente, y Cali no es la excepción.
"Lo que buscamos es que los embarazos sean responsables, deseados y en edades no tan tempranas. El año pasado 260 niñas menores de 14 años tuvieron bebés", agrega.
"Sabemos cómo esta situación puede afectar sus proyectos de vida, su salud física y mental, la situación del niño mismo y lo que significa para la sociedad", dice Quebradas.
El año pasado 6.100 jovencitas entre los 10 y los 19 años se convirtieron en madres. Ellas representan un 20,4 por ciento del total de los nacimientos en el 2011.
Graciela Abuchar, de la Línea de Salud Sexual, dice que "este espacio aborda temas que interesan a los jóvenes para que tengan una mirada aterrizada de sus proyectos de vida, pero con el ingrediente de la ternura y el amor".
Se busca reflexionar con los jóvenes que los embarazos no son situaciones de uso exclusivo del preservativo, sino del contexto social en que se dan, "tenemos claro que algunos son de niñas víctima de abusos que quieren salir de la casa o de aquellas que buscan arreglar su vida", dice.
No es la única problemática que se atiende y preocupan las enfermedades de transmisión sexual. Abuchar refiere que hasta hace 10 años se daba una proporción de una adolescente por 10 muchachos contagiados con VIH.
"Hemos llegado a una proporción casi pareja de un caso en jóvenes por uno de jovencita. Estas enfermedades no distinguen edad, sexo ni estrato, lo único que distinguen es la irresponsabilidad en el manejo de su sexualidad y es un factor determinante para que estas enfermedades, incluyendo el VIH, se disparen", dice.
Lo más dramático es que cada vez es más precoz el inicio de la sexualidad. En Cali, dicen las expertas, se han encontrado casos desde los 10 y 11 años

"Well, for the past week or so I've been so tired and bloated. My head hurts, my nose is stuffy, I'm nauseous. It seems like I have to go to the bathroom all the time. On top of that, my breasts really hurt and feel swollen. Maybe I'm just getting my period."
"Isn't your cycle really irregular? Maybe you're just stressed out because of your trip."
"Yeah, but I don't think I'm that stressed."
"You know, when my aunt was pregnant, I remember her feeling the same way you just described. She called it morning sickness. Hey, maybe you're pregnant!"
"Very funny, Laura. That's impossible."
"Well, wait. Didn't you tell me you slept with Diego the night before graduation?"
"Yeah, but that was my first and only time. Besides, like I said, my period is so irregular, there's no way I could have gotten pregnant."
"Not to scare you or anything, but why don't you take a home pregnancy test so you can be 100% sure. I promise I'll go with you to the drugstore and won't tell anyone, okay?"
"What!!! Well, okay, but you have to be there to see the results."
“I wonder if mom knows she's pregnant.... I've been here nine weeks, she's got to know soon.
It seems like only yesterday I was a tiny blastocyst, traveling down her Fallopian tube and implanting into her uterine wall. I'm about half an inch long now, and I weigh a little over a gram. I'm growing like crazy! My ears, nose, mouth, and limb regions are popping out and the cerebral hemispheres of my brain are well defined. My hindbrain, which will regulate my heart, breathing, and muscle contractions, is really starting to develop. So is my pituitary gland, which will regulate the rest of my glands once they develop. Although my eyes have formed, the eyelids are still fused so I can't blink yet. My heart has been beating for a few weeks and is now separating into four chambers. My diaphragm, stomach, kidneys, and intestines are developing too. Although my gonads have formed, they still haven't differentiated into either testes or ovaries, but I know genetics has destined me to be a beautiful, healthy girl. I'm receiving my nourishment through the yolk sac now, but in about two to three weeks, my mom's placenta will take over completely. I feel so sorry for her right now. The high levels of HCG and progesterone circulating through her blood are making her nauseous, but I know she'll look past that once she knows I'm in here. Even though these placental hormones are making her feel bad, I need them so her body will not reject me and instead allow me to continue developing. Mom doesn't know it, but the folic acid in her daily multivitamin is helping my nervous system develop, her calcium is helping my bones grow strong.
I hope mom will be happy when she finds out I'm here”.

UNIDAD 10. “Lucas está corrido, se siente deprimido”
Lucas Upegui walked through the mall looking for the Pasteur drugstore. He had been feeling depressed lately and was not looking forward to starting the semester in this mood. The previous day, while eating dinner, he had spoken to his mother about his feelings.
"You know, Mom, I just don't feel like doing anything these days. I'm feeling depressed, don't like hanging out with my friends or going to the gym. I even called in sick to work today. I'm not sure I really want to go back to Medical School this semester. Maybe I should take some time off."
Mrs. Gomez was concerned. This did not sound like Lucas. He was usually an upbeat guy, liked his job at the software company, and always looked forward to the start of the school year, especially since he was hoping to graduate in the 2012.
"Perhaps you should visit my doctor, and he can give you something to make you feel better," she told him. She herself was under a doctor's care and was taking a prescription drug to help control her depression.
"You know how I feel about taking a lot of meds," he said. "Those anti-depressants are so unnatural and can have side effects like sleeplessness, agitation, and headaches. I saw a news report last week about a new dietary supplement for depression. Maybe I should go to the natural food store and check it out." Lucas headed out the door and toward the mall.
As Lucas browsed the aisles of the Pasteur drugstore, a sales clerk approached him.
"May I help you find something?" the clerk asked.
"I'm looking for something that might help with depression," replied Lucas. "I saw a news report about a supplement that is now being imported from Europe."
The clerk led Lucas to the next aisle and handed him a flyer on SAMe.
"This is a new product that we have just started carrying. It has been used in Europe for years. Why don't you take a few minutes to look over the pamphlet? I'd be happy to answer any other questions."

Caso 7 Accidentes en el hogar

UNIDAD 7 ACCIDENTES EN EL HOGAR
This is a 3 year old male who is brought to the emergency room after suffering burns to the right arm when a hot cup of coffee spilled on him. His family applied gel from an Aloe plant to the burn before coming to the ER. He is otherwise healthy and his immunizations are up to date.
Exam: VS T 37.5, P120, R20, BP (unobtainable, crying), oxygen saturation 100% in room air. Weight 15 kg. He is alert and active, in moderate distress. His right forearm appears mostly red, and is painful upon palpation. Blanching is apparent on palpation. One smaller intact blister and one large ruptured blister are present on the dorsal aspect of the right forearm. The blistered surface area is estimated to be 2% and the non-blistered red area is estimated to be 2% for a total body surface area (TBSA) of 4%.
The physician recommends an IM dose of morphine, but his parents decline this, so acetaminophen with codeine is given instead. Cool, sterile, saline-soaked gauze is applied to the wound surface and the open areas are gently cleansed with saline. Tissue from the ruptured blister is removed and the intact blister is flimsy so it is drained and removed. Silver sulfadiazine cream is applied to areas of partial thickness burns and the burns are dressed. He is discharged with instructions for the parents to gently cleanse the burns with mild soap and water, reapply the antibacterial cream, and change the dressing each day. His burn healed completely without complications in five weeks.

Sixty minutes before the race, Jim was sitting quietly on the bank of the Cauca River. He was visualizing the race he was about to row. Two thousand meters of intense physical activity, pushing his body to the very limits of its capabilities. But sitting there, he was calm and relaxed, mentally willing his heart rate and respiratory rate down. He had done his stretching and warm-up exercises, but his heart rate was now just 65 beats per minute and he was breathing 12 breaths per minute. His body temperature was 37° C (98.6° F). He was well hydrated. His weight was 180 pounds.
That was an hour ago. Now, he was sitting in the bow seat of the Men's Varsity Eight. In lane four on the starting line, he could see two boats to his left and three boats to his right. The rowers all looked bigger than him and his crew, but then they always did. The starter on the shore was saying something over the loudspeaker but Jim wasn't paying attention. He was concentrating on being ready and was listening to his coxswain. These last few seconds before the race were the most stressful--you could feel the tension in the air. He knew that all 48 rowers and even the six coxswains on that starting line were feeling the same as he was. He was sweating although the air was cool. His heart rate was now 85 beats per minute and he was breathing 18 breaths per minute. He felt a nervous excitement. His mouth was dry. He took one last sip of water.
"All hands are down," he heard the starter say. He tensed his muscles in his starting position.
"Et vous prez... PARTE!" which was French for "Are you ready... row!"
Three short strokes to get the 60-foot-long shell moving, and then 20 strokes at maximum power. His crew was rowing 39 strokes per minute and water was flying everywhere. It seemed like he could hear everything--coxswains yelling, rowers grunting, oars and rigors banging. Mostly he heard himself breathing. He was putting all of his strength into each stroke, knowing that after those first 20, the pace and the power would come down some.
At the end of that first minute, Jim's heart rate was 201 beats per minute. He was taking two breaths per stroke, fast and forced. Their stroke rate was now 34 strokes per minute. He was sweating more now. His body temperature was 37.5° C. His muscles hurt--they felt like they were burning.

It seemed like he could hear everything--coxswains yelling, rowers grunting, oars and rigors banging. Mostly he heard himself breathing. He was putting all of his strength into each stroke, knowing that after those first 20, the pace and the power would come down some.
At the end of that first minute, Jim's heart rate was 201 beats per minute. He was taking two breaths per stroke, fast and forced. Their stroke rate was now 34 strokes per minute. He was sweating more now. His body temperature was 37.5° C. His muscles hurt--they felt like they were burning.

NORMAL AND PATHOLOGICAL CONDITIONS
Peter Santana is an 18-year-old man who fatigued quickly when he was taking exercise; yesterday he complained of lassitude.
At 13:20 on the last Friday in September, he was invited to spend the weekend at “The good life spas.” Today, all services offered by this booming recreational and sport center are completely full by an exaggerated number of people in search of better physical conditions.
“This is what I call a healthy body”, Peter says, who before a mirror of the room boasts of his superior train big muscles just because Erika, a slender and young student of the modeling Top Forma Academy.
-“Honey, look! Enjoy your sight! This is a normal man, I bet my body mass index (BMI) is normal and it corresponds to a healthy life.”
The gym environment is full of music and the people are working out conducted by an instructor. Suddenly, an anguished voice is heard over the loudspeakers at the end of the room: “Your attention, please! Your attention, please! This is an emergency! Peter Santana has just been gotten out of the pool in an unconscious state, we need a physician urgently.”
Among those present, was Maurice, a medical student who leans quickly over the body that lies on the floor and says to his companion: “Buddy, this guy doesn’t breathe, he’s blue and he seems not to have vital signs; I can’t feel his pulse, apparently he drank the whole pool, he’s frozen.
Fifteen minutes later after a series of resuscitation maneuvers performed by Maurice, he exclaims delightedly: “Thank Heaven! In this moment he’s got pulse: 100 per minute, blood pressure, 130/90; temperature 370 C, and breathing 30/min. After this, an ambulance rushed him to The Vegas Clinic.

“Nancy, let’s trust in God”
In a calm morning in the Eternal Spring City, Nancy, as usual, walks quickly toward the Divine Incarnation Church convinced that her aunt Filomena’s predictions are not true.
“Get ready, lass, pray a lot that the end of the world isn’t far away now, these climatic changes are leading the world to an ecological disaster,” she used to repeat every morning when she left for the church.
Lost in thought, she did not notice the recommendation that a police assistant did her and who told her to move some meters further away: “Ma’am, we’re told that there is a car bomb with 1,000 kilos of dynamite in front of the church and it’s being defused, move away, please.” The young policeman hasn’t finished his words when a explosion is heard, due to it, cars were blown to pieces as well as people, buildings were collapsing in the eyes of onlookers and the pain exclamations of the injured.
.... What appeared a calm morning of May, it is filled of noisy sirens now, people run terrified over the rubble and mutilated bodies by the shock wave and the fall of walls.
“Buddy, let’s organize, evaluate the injured, let’s do a triage and let’s proceed to revive the people who really need it; let’s decide who must be taken to the hospital.”
“I bet these priests don’t have an evacuation plan in case of disasters and less the people of this neighborhood, be on the alert to the fire that there is in that house!”
